Instructions

Scroll down the left-hand column to link to all of the forms and information you need to enter the Fall Games.  This schedule is divided into sections by sport. Each link (to the left) includes a schedule for that sport, the rules for that sport and the entry forms for that sport.

Make copies of any pages you need
to complete & submit.

NOTE:
Everyone MUST complete and submit a

• Coach’s Card,
• Athlete Roster, and
• Sport-specific
  • Entry forms &
  • Copies of medical/release forms

Click on the link or links (left column) that you need, print the pdf pages, complete the forms and then submit.

SUBMIT FORMS TO:
The forms should be mailed or hand delivered to the event director that is listed on the schedule before or by the deadline.
