Thank you for choosing to volunteer with Special Olympics Oklahoma! This page is for groups with 5 or more individuals. Please complete for businesses, civic, church and school groups who volunteer together providing manpower to run the events.

This application will ask you to designate an individual to be the contact person for your group. If anyone in your group is able to answer “yes” to any of the following questions below, they are required to fill out a complete individual “Class B” application as well.

  • Do you use illegal drugs?
  • Have you ever been convicted of a criminal offense?
  • Have you ever been charged for neglect, abuse, or assault?
  • Has your driver’s license been suspended or revoked in any state?

We also require every member of your group to fill out a Communicable Disease form below.
