Coaches and Parents/Guardians we are now going to the next online phase of registration. Instead of printing out, filling out, scanning, naming and emailing medical forms you will now go to your portal and submit the Athlete and Unified Partner Medical Release Forms. Young Athletes will complete the Athlete forms.
Coaches you are able to complete a medical release for athletes and unified partners but note that if an athlete or unified partner is not an adult that can sign for themselves you will need to have the parent e-sign the form. Nothing is to be printed it is all electronic.
Please note. When a you or a parent completes the portal they can put their preferred language. The portal and forms will then be in that language and when you view your delegations medicals/releases will be in your proffered language.
Parents it is easier for you to create a portal and add your child to your portal as an athlete or unified partner. This will give you access to your child’s medical release and notification when the medical release needs to be updated but also in the future it will provide all your child’s competition event details.
